Child deaths - information for professionals

When the death of a child is reported in Derbyshire, it is our duty to make sure that there is a review of the circumstances of the death.

The review is carried out by a child death overview panel, made up of representatives from the organisations which are part of the our safeguarding children's board.

The panel reviews all deaths of children and young people aged under 18 (excluding those babies who are stillborn).

This involves:

  • collating information from professionals and family members
  • letting families know what future actions will be taken as a result of the review
  • deciding which factors contributed to the child's death and whether their death might have been prevented
  • deciding if action can be taken to prevent similar deaths and making recommendations
